Introduction

The Talus of Vilex, TOV®, implant is used to correct the Valgus position of the calcaneous. The implant is conically shaped to conform to the sinus tarsi and is cannulated to facilitate insertion and positioning. It is made of implant-quality titanium alloy and has other novel design features unique to Vilex.

Five sizes are offered. They are color coded to match the instruments, which consist of a probe, sizers, and a hex driver and grabber retriever.

 


 

 

 


 

 



   

Surgical Protocol

Step 1

Make an incision over the sinus tarsi no longer than 3 cm. Avoid the cutaneous nerves. Incise the fascia without damaging the ligaments until the entrance to the sinus tarsi is reached. Minimal dissection is necessary and a sinus tarsectomy is not called for.


       
   

Step 2

Place the TOV® Probe (black handle) into the sinus tarsi as deep as feasible without any forward thrust.

Rotate the probe to dilate the sinus.
       
   

 

Step 3

With the probe in position, insert the guide wire through the probe and push the wire the sinus until it tents the skin on the medial side. Do not penetrate the skin.

 

       
   

Step 4

Remove the probe, and slide the smallest sizer #1 (color: blue) over the guide wire into the sinus tarsi. If the desired correction is not achieved with a particular sizer, replace the sizer with the next larger size until calcaneuos reverts to perpendicular or 2 to 3 degrees valgus.

       
   

Step 5

The sizer is then removed and the corresponding TOV® implant is inserted, advanced with hexagonal screwdriver until it is securely embedded.

The surgeon should note that the TOV® thread is not a cutting type. In advancing the implant, the screwdriver slides the implant into the sinus tarsi. However, if the surgeon decides to retrieve the implant or to reposition it laterally, the screwdriver may not necessarily be capable in moving backward.

     

 

   

Step 6

The TOV is now in place..

 

 

Step 7 - To Retrieve or Adjust the Implant:

To this end, the TOV® implant possesses an internal thread that can be used to engage a threaded retriever grabber, which can provide a mechanism for pulling the implant laterally.

To retrieve the implant, locate the TOV® cannulation center using a guide wire and then engage the screwdriver. With the screwdriver in plane, retrieve the guide wire and insert the grabber through the cannulation of the driver.

The grabber is then pushed into the TOV® implant, engaging the left-handed thread by turning the grabber counterclockwise. With one hand turning the driver and another hand pulling on the grabber, the TOV® is repositioned or retrieved altogether as desired.
